Admission Requirements for SHS
Admission Guidelines
For regular applicants:
- Completely filled-out Application form. You may click here to apply online
- Current school identification card (ID)
- Application fee (if applicable)
Additional requirements for Junior High School completers who did not enroll the following school year in Senior High School:
- Original and photocopy of Grade 10 Report Card
- Photocopy of certification from the high school principal stating that the applicant’s Form 137-A is still in the school file and has not been forwarded to another school.
Additional requirements for foreign nationals:
- Alien Certificate of Registration (ACR) or Special Study Permit (SSP)
- Original and photocopy of Passport
Additional requirements for graduates of Non-Formal Education Accreditation and Equivalency or finishers of Alternative Learning System Accreditation and Equivalency:
- Certificate of Rating (with passing marks in all subjects)
- Learner’s Permanent Record (AF-5) with eligibility to senior high school
Additional requirements for foreign applicants who completed junior high school abroad except from Philippine schools under DepEd:
- Scholastic Records, duly authenticated by the Philippine Foreign Service Post located in the student applicant’s country of origin or legal residence; if the country or territory of origin is already a member of the Apostille Convention, authentication of the Philippine Foreign Service is no longer necessary. - Certificate of completion of in Grade 10 (junior high school / grade 10 or its equivalent)
- Photocopy of applicant’s passport bio-page, latest admission and authorized to stay.
IMPORTANT REMINDERS:
- Applicants with at least 85% General Weighted Average (GWA) and final grades in Science, Mathematics, and English in Grade 9 are exempted from taking the Mapúa Scholastic Aptitude Examination (MSAE).
- Applicants who attended Non-Formal Education Accreditation and Equivalency or completers of Alternative Learning System Accreditation and Equivalency shall be required to take the MSAE.
- All applicants who are interested to study senior high school in Mapúa, regardless if they are exempted or not in taking the exam, shall provide all admission requirements.
- Applicants exempted from taking the MSAE may confirm their slots by paying the non-refundable reservation fee of Php 5,000 online or at the Mapúa Treasurer’s Office starting August 19, 2019.
- The application is considered official once a receipt has been issued (for MSAE takers).
Enrollment Requirements for incoming senior high school:
For regular enrollees:
- Original Grade 10 report card with the applicant’s eligibility for admission to senior high school duly signed by the school principal
- Original Certificate of good moral character (dated not earlier than February of the completion year) from the high school (with school seal)
- Original birth certificate printed on PSA paper
- Fully accomplished SHS Student Data Sheet / SHS Enrollment form
- 1 pc. ID picture (plain background), size 1 ½” x 1 ½”
- One (1) long brown envelope to contain all of the above
